CPT Code for TOVA, Conners CPT 3 and QbCheck

There is no product-specific CPT code for the TOVA test, the Conners CPT 3, QbTest, or QbCheck. Procedure codes describe the service performed, not the brand of test. The same product can therefore be reported under different codes depending on who administered it, whether it was part of a battery, and what interpretation followed. The codes clinicians typically consider are 96146 (a single automated test with an automated result), 96136–96139 (administration and scoring of two or more tests), and 96130–96133 (evaluation services). This page answers the product-specific questions. For the full code-by-code guide, start with our ADHD testing CPT code guide.

Why there’s no “TOVA code” or “QbCheck code”

The testing codes are written around the work done. APA Services’ 2026 guide says 96146 “is used to describe the use of a single, automated test … administered via electronic platform (e.g., computer, iPad), that produces an automated report.” The code exists to separate “a limited, single psychological or neuropsychological automated test” from “a robust battery of tests” [1]. Nothing in the descriptor names a product.

The vendors don’t assign codes either:

  • On the product pages we reviewed on September 27, 2026, Qbtech, The TOVA Company, and MHS did not list billing codes for QbTest, QbCheck, T.O.V.A., or the Conners CPT 3 Online [2][3][4].
  • BrainTrain (IVA-2) sends users to APA Services “for the most up to date billing codes for tests” [5].

So the question to ask is not “what’s the code for the TOVA?” It’s “what did I do, and which descriptor matches it?” For how these same products compare on setting, hardware, age range, and FDA status (not just billing), see our full CPT comparison: TOVA, Conners CPT 3, QbCheck, and IVA-2.

Common scenarios and the codes they point to

These scenarios illustrate how the descriptors apply. They are not a coding determination for any product.

What happened Descriptor that may match Key conditions
One computerized CPT, taken without a psychologist, QHP, or technician administering or scoring it, producing an automated report 96146 Single automated instrument; “automated result only”; billed once even if more than one automated test is given [1][6]
The CPT is one of two or more tests, administered and scored by a technician 96138 (+96139) Descriptor says “two or more tests”; time-based [1]
The CPT is one of two or more tests, administered and scored by the psychologist or QHP 96136 (+96137) “Two or more tests”; time-based [1]
Integrating CPT results with other data, interpreting, writing the report, giving feedback 96130 (+96131) psychological, or 96132 (+96133) neuropsychological Reported with the administration codes when a battery is given [1]
ADHD rating scale scored and documented 96127 Per standardized instrument; a separate service from the CPT [7]

Two points from CMS are worth knowing. The testing code range “96105-96146” includes codes that “describe tests performed by technicians and computers,” and 96146 “is billed once regardless of the time spent completing the tests” [6].

Where does “96312” come from?

In a May 2023 AAPC forum thread, a coder asked what codes to use for QbTest. One reply said: “96312 is the CPT code we use for our QB testing” [8].

96312 falls outside the psychological and neuropsychological testing range that CMS describes (96105–96146) [6], and it does not appear in APA Services’ 2026 list of testing codes [1]. We could not find any authoritative source that supports using 96312 for a CPT. Don’t copy a code from a forum post. Match the service to a published descriptor, then confirm with the payer.

Can a single TOVA be billed as 96138?

This question comes up often. In a February 2023 AAPC thread about TOVA, a reply said that 96130 can be billed together with 96136/96138 “per CPT manual,” and recommended getting payer pre-approval [9].

The descriptors for 96136–96139 cover administration and scoring of “two or more tests” [1][10]. APA Services also advises against reporting 96146 when two or more tests are given or when a professional or technician administers the test [11]. The descriptor therefore turns on whether the CPT was one test given alone or part of a battery, and on who administered it.

Product notes

T.O.V.A. 9. A visual and auditory CPT run on a proprietary USB device with a microswitch. Tests are 21.6 minutes, or 10.8 minutes for ages 4 to 5.5 [12]. It is in-clinic hardware, so a technician or clinician is usually present. Whether their involvement is “administration and scoring” under 96136/96138, or whether the test fits 96146, depends on your workflow and the payer.

Conners CPT 3 Online. Administered and scored online through MHS’s platform. MHS says it supports “both local and remote administration” [4]. For how it compares feature-by-feature to a fully remote option, see AxonCPT vs. Conners CPT 3.

QbTest and QbCheck. QbTest is in-clinic. QbCheck can be taken remotely through Qbtech’s patient portal [2]. Both report attention, impulsivity, and activity measures [2]. For how the two differ, see QbTest vs. QbCheck.

Will the payer cover it at all?

Coding is only half the question. Some payers treat standalone CPTs as unproven for ADHD. Aetna’s CPB 0426 (last reviewed July 7, 2026) lists “computerized tests of attention and vigilance (continuous performance tests)” as experimental, investigational, or unproven for ADHD [13]. Before choosing a code, check the payer’s policy. Our overview of whether ADHD CPT testing is covered by insurance compares Aetna, Evernorth/Cigna, Florida Blue, and Medicare.

Credentials matter too. PMHNPs should confirm that the payer allows them to report testing codes. See can nurse practitioners bill 96130?

A pre-billing checklist

  1. Describe the service. How many tests? Who administered and scored them: a computer, a technician, or you? Was there a separate interpretation and report?
  2. Match the descriptor using APA Services’ guide [1] and CMS’s FAQ [6].
  3. Check the payer policy for the indication, including whether CPTs are considered unproven [13].
  4. Check provider eligibility for your credential and the payer.
  5. Document the reason for testing, the results, and how they informed the decision. WPS, a Medicare contractor, requires that “the medical record must document the reason the tests are being performed” [14].
  6. Confirm with a certified coder before you submit.

How this looks in practice

A prescriber using a remote CPT for titration usually works through the same questions:

  1. The test. For example, with objective ADHD testing with AxonCPT, the patient opens a link and completes the test in about 16 minutes on their own device. The report is generated automatically.
  2. The coding question. Is this a single automated test with an automated result, or one part of a larger evaluation? The answer depends on your full workflow. We don’t make claims about which code, if any, applies to AxonCPT or any other product. Ask your coder and payer.
  3. The clinical record. Document why you tested, what changed, and what you decided.

You can see what the report looks like and judge how it fits your documentation.

Frequently asked questions

What is the CPT code for a TOVA test? There isn’t a TOVA-specific code. Depending on the service, clinicians consider 96146 for a single automated test, 96136–96139 when the TOVA is part of a battery of two or more tests, and 96130–96133 for evaluation services [1]. Verify with the payer.

What is the CPT code for QbTest or QbCheck? The same logic applies. No code is specific to Qbtech products. Treat forum suggestions such as “96312” with caution: that number falls outside the testing code range CMS describes [6][8].

Can 96146 and 96136 be billed together? APA Services says not to report 96146 when two or more tests are given or when a professional or technician administers the test [11]. CMS describes scenarios in which technician and computer-administered tests are both billed when the results are integrated [6]. Payer edits vary, so check before billing.

Does the vendor tell you what code to use? Generally no. BrainTrain refers users to APA Services [5], and the Qbtech, TOVA, and MHS product pages we reviewed listed no codes [2][3][4].
