Cost of ADHD Testing for Clinicians: 2026 CPT Prices

For a clinician, the cost of ADHD testing with a continuous performance test (CPT) ranges from $0 on a free allowance to about $10–$15 per test on per-use or per-credit pricing, and from $68 per test on QbCheck’s published entry plan. Hardware-based systems add an upfront purchase: $1,095 for a T.O.V.A. 9 system, or $975–$4,795 for an IVA-2 license kit. Several vendors don’t publish prices at all. Below are the list prices vendors publish, what each includes, and worked cost examples with the arithmetic shown, so you can budget for objective ADHD testing with AxonCPT or any other platform.

Why clinician cost is hard to find

Search “ADHD test cost” and most results are clinic pages quoting what a patient pays, often a few hundred dollars for a whole evaluation. That tells you little about what the test itself costs your practice. The clinician-side price depends on three things:

  • Pricing model. Per-test credits, per-use scoring fees, hardware plus credits, multi-year software licenses, or subscriptions.
  • Volume. A practice that runs one baseline test per new patient has very different costs from one that retests at every dose change.
  • Setting. Some products are in-clinic only. Others add a separate fee for remote administration.

If you prescribe but can’t bill psychological testing codes with every payer, the test is often an unreimbursed overhead or a self-pay item. That makes the per-test price matter more. See can nurse practitioners bill 96130? for how payer rules affect PMHNPs.

What does each test actually cost per patient?

List prices only become useful once you apply them to a caseload. The examples below use one simple assumption: 50 tests in the first year, for example 10 patients tested five times each. Each figure is list price only and excludes tax, shipping, computers, staff time, and any discount.

AxonCPT

  • Packs only: 2 × $149 (40 tests) + 2 × $49 (10 tests) = $396, or $396 ÷ 50 = $7.92 per test.
  • Unlimited: $79 × 12 months = $948 for the year, regardless of volume.
  • The free plan covers up to 2 tests a month (up to 24 a year) at $0.

QbCheck

  • From $68 × 50 = from $3,400, or from $68 per test. Credits expire 12 months after purchase.

T.O.V.A. 9

  • $1,095 system (includes 5 credits) + 45 × $15 = $675 in credits = $1,770, or $1,770 ÷ 50 = $35.40 per test in year one.
  • After the system is paid for, the marginal cost is $15 per test before bulk discounts.

Conners CPT 3 Online

  • 50 × $10 = $500, or $10 per test. The first order must be at least 15 uses ($150). MHS’s FAQ says a use is consumed the first time an Assessment Report is generated, and Progress Reports don’t consume a use (MHS FAQ).

IVA-2

  • Tests only: Standard kit $975 (5 tests) + one 50-test pack $825 = $1,800 (55 tests available).
  • With a narrative report for each test: add one 50-report pack ($825) = $2,625, or $2,625 ÷ 50 = $52.50 per test.
  • Add $695 if you need the Remote Testing License.
  • The kit is a 3-year license, so charging all of it to year one overstates the annual cost. Renewal is $775 for Standard.

QbTest, MOXO, Creyos: not calculable from public list prices.

Costs that don’t show up on the price list

A per-test price isn’t the whole cost. Budget for these too:

  • Hardware and computers. T.O.V.A. 9 uses a proprietary USB device and runs on Windows or Mac computers; its requirements page says it isn’t compatible with Chromebooks, iPads, or Android devices (TOVA computer requirements). IVA-2 requires Windows 11 and isn’t compatible with macOS (BrainTrain).
  • Remote administration. Conners CPT 3 Online supports remote administration, while K-CPT 2 and CATA are local only (MHS). IVA-2 remote testing is a $695 add-on, and BrainTrain requires that the examiner can view the person being tested throughout the test (BrainTrain remote testing). QbCheck at home requires a Mac or Windows laptop with a built-in webcam; Qbtech’s FAQ says smartphones, Chromebooks, Surface devices, and tablets aren’t compatible (QbCheck FAQ).
  • Staff and room time. In-clinic tests need a room and often a proctor for 20+ minutes. At about 21.6 minutes per T.O.V.A. test and 20 minutes for QbTest (vendor figures), that time is part of the cost.
  • Expiring credits. QbCheck credits are valid for 12 months. AxonCPT packs don’t expire. Other vendors’ pages reviewed don’t state an expiry.

Does insurance offset the cost?

Sometimes, not reliably. Coverage depends on the payer, the code, and who performs the service. Some payer policies are skeptical: Aetna’s Clinical Policy Bulletin 0426 on ADHD lists “computerized tests of attention and vigilance (continuous performance tests)” among assessments it considers experimental, investigational, or unproven (Aetna CPB 0426, accessed September 27, 2026). And PMHNPs may not be able to bill the 96130-family codes with every payer. Code definitions come from the AMA and APA Services’ testing billing guide (APA Services). Verify coverage and code choice with each payer before you rely on reimbursement.

A note on regulatory status

FDA’s 510(k) database lists clearances for QbCheck (K143468), QbTest (K133382 and earlier), and T.O.V.A. (K173915 and earlier). BrainTrain states that its remote products “are not medical devices.” AxonCPT is not marketed as an FDA-cleared device; it describes itself as objective measurement to supplement clinical judgement. If your institution or payers have requirements about device status, check them directly. It’s a separate question from price.

When another option may fit better

Price is one factor. Another product may suit you better if:

  • You need a test for young children. T.O.V.A. publishes a 10.8-minute visual test for ages 4 to 5.5, and MHS offers K-CPT 2 for ages 4 to 7.
  • You want motion tracking. QbTest and QbCheck measure motor activity alongside attention and impulsivity.
  • You need auditory and visual modalities. T.O.V.A. and IVA-2 both offer auditory testing.
  • You need an FDA-cleared device for institutional or payer reasons.
  • You already own the hardware or license. If you’ve paid for a T.O.V.A. system or IVA-2 kit, the marginal cost per test may already be low.
  • Your practice depends on a specific normative sample or published research base for a particular product.

How this looks in practice

  1. Estimate your volume. Count new evaluations per month and how many follow-up tests each titration patient typically gets.
  2. Match the setting. If most visits are telehealth, rule out in-clinic-only products or price the remote add-on.
  3. Run the arithmetic. Multiply your yearly volume by per-test price, add fixed costs (hardware, licenses, remote add-ons), and divide by volume.
  4. Test the workflow before committing. With AxonCPT you send a link, the patient completes the ~16-minute test on a phone, tablet, or desktop with no app or login, and the report is available as soon as they finish. See what the report looks like before you sign up.
  5. Document the result alongside the interview and rating scales. A CPT supports clinical judgment; it doesn’t diagnose ADHD on its own. A 2024 systematic review concluded CPTs “should not be used in isolation as a diagnostic test” in adults but may help as part of a comprehensive assessment [1].

Frequently asked questions

How much does an ADHD test cost a clinician per patient? From published list prices, one test costs $0 on AxonCPT’s free allowance, $7.45–$9.80 on AxonCPT packs, $10 on Conners CPT 3 Online, $15 per T.O.V.A. credit after the $1,095 system, and from $68 on QbCheck. Hardware, licenses, and minimum purchases change the real per-patient figure.

Which CPT vendors don’t publish prices? On September 27, 2026 we found no public price for QbTest, MOXO, or Creyos on the vendor pages we reviewed. Ask each vendor for a written quote, including renewal terms.

Is a cheaper test less accurate? Price tells you nothing about validity. Compare normative samples, published studies, and whether the test fits your population and setting. Clinicians in a UK study valued an objective test but were “conscious of the additional costs” [2], so weigh both.

Can I charge patients for the test? Practices often bill insurance where allowed or charge self-pay. Rules depend on your payer contracts and state, so check them before setting a fee.
